Hello Anne Arundel County Volunteers
It was good seeing you at last Friday's County meeting. See the attached outline of the current Accident and Health insurance plan provided by the Anne Arundel County Fire Department (Policy Number PRCO-87018-MD10058). Benefits of the plan apply to members of County volunteer emergency organizations while performing a covered activity of your company. The current plan covers the period March 1, 2025 through Feb. 28, 2026. The plan is not a substitute for Workers' Compensation or any other health insurance coverage you may have.
First Responder Assistance Program (FRAP)
The County policy includes confidential access 24/7 to experienced health clinicians for help with PTSD and other Mental Health concerns. This service is provided at no cost to both you and your household members. You do not have to file a Notice of Claim to use FRAP. See more at www.providentins.com/FRAP or call 855-207-1747.
Claims
You may submit the First Notice of Claim online using the fill-able and printable form found on a dedicated claims website at www.providentclaims.com. This is used in the event of an injury or illness you believe was incurred while acting as a member of your volunteer company. The First Notice should be signed by an officer or a designated authorized member of your company and submitted directly to:

You may also submit the First Notice of Claim using a paper form. To request a pdf of this form call or email Claims at the email address or number above.
The First Notice of Claims is the initial notice to Provident. It runs multiple pages so enter as much information as you can. They will follow-up with you for any additional information needed to document and process your claim.
Volunteers are also encouraged to file a claim with the County's Workers' Compensation carrier, if applicable. The Provident Accident and Health plan is not a substitute for Workers' Compensation.
IMPORTANT: Members of volunteer emergency organizations located within the City of Annapolis are not covered by this County-owned policy. If you are a member of an Annapolis city volunteer organization or the City Fire Department and interested in obtaining your own policy, please contact my office for a proposal.
Please contact me for any questions or other assistance you may require. I am also available to meet with your officers and members to review the program.
I am also happy to provide proposals for other benefits Provident offers for volunteer organizations like group term life insurance, 24-hr. accidental death and dismemberment and critical illness insurance. Contact my office for a proposal.
Thank you for the continued opportunity to be of service.
